Complaint Filing Checklist don't forget to include the following information when filing your complaint
One (1) original completed and signed complaint form

One (1) copy of the completed and signed complaint form

Two (2) copies of all supporting documentation

* DO NOT * send originals of supporting documentation

Use either the standard or fillable forms to file your complaint, or fill out our online form to have the complaint form mailed to you.

Recovery Fund Forms

If you haven't filed a complaint, you may not be eligible to file a Recovery Fund claim.
